Support Physical Therapists in implementing individualized outpatient rehabilitation programs within patients’ homes.
Carry out therapeutic exercises, gait and mobility training, and additional rehabilitation interventions under PT direction.
Maintain accurate and timely documentation of patient progress using electronic or paper charting systems.
Educate patients and caregivers on prescribed exercises, mobility techniques, and home safety measures.
Promote a safe, professional, and patient-centered environment during home visits.
Collaborate with Physical Therapists and interdisciplinary healthcare teams to ensure coordinated, high-quality patient care.
Provide care for diverse patient populations, including post-surgical recovery, motor vehicle accident injuries, and chronic medical conditions.
Utilize rehabilitation techniques such as trigger point therapy, neuromuscular re-education, and manual therapy to enhance patient recovery and functional outcomes.
Requirements
Associate degree from an accredited Physical Therapist Assistant (PTA) program.
Active state PTA license or eligibility for licensure.
Strong understanding of outpatient rehabilitation practices, anatomy, physiology, and medical terminology.
Previous experience in home-based outpatient therapy or related healthcare settings preferred.
Ability to work independently, prioritize tasks, and manage patient visits across assigned territories.
Excellent communication, interpersonal, and patient education skills.
