Job Description

Responsibilities

  • Support Physical Therapists in implementing individualized outpatient rehabilitation programs within patients’ homes.
  • Carry out therapeutic exercises, gait and mobility training, and additional rehabilitation interventions under PT direction.
  • Maintain accurate and timely documentation of patient progress using electronic or paper charting systems.
  • Educate patients and caregivers on prescribed exercises, mobility techniques, and home safety measures.
  • Promote a safe, professional, and patient-centered environment during home visits.
  • Collaborate with Physical Therapists and interdisciplinary healthcare teams to ensure coordinated, high-quality patient care.
  • Provide care for diverse patient populations, including post-surgical recovery, motor vehicle accident injuries, and chronic medical conditions.
  • Utilize rehabilitation techniques such as trigger point therapy, neuromuscular re-education, and manual therapy to enhance patient recovery and functional outcomes.

Requirements

  • Associate degree from an accredited Physical Therapist Assistant (PTA) program.
  • Active state PTA license or eligibility for licensure.
  • Strong understanding of outpatient rehabilitation practices, anatomy, physiology, and medical terminology.
  • Previous experience in home-based outpatient therapy or related healthcare settings preferred.
  • Ability to work independently, prioritize tasks, and manage patient visits across assigned territories.
  • Excellent communication, interpersonal, and patient education skills.
  • Dedicated to helping patients restore mobility, increase independence, and improve quality of life in the home setting.
