Job Description

Responsibilities

  • Provide high-quality, hands-on patient care under the direction of physicians and registered nurses.
  • Perform ongoing patient assessments, monitor clinical status, and promptly communicate changes in condition to the healthcare team.
  • Administer medications, injections, and prescribed treatments, including botulinum toxin therapy when indicated.
  • Assist in developing, implementing, and evaluating individualized patient care plans to support optimal health outcomes.
  • Support patients with activities of daily living, including personal care, mobility assistance, nutrition, and overall comfort.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver coordinated, patient-centered care across clinical, geriatric, and assisted living settings.
  • Maintain accurate, timely, and compliant medical records and clinical documentation.
  • Educate patients and family members on treatment plans, disease management, preventive care, and wellness strategies.

Qualifications

  • Previous patient care experience required; experience in senior care, geriatrics, or assisted living settings strongly preferred.
  • Proficiency in patient assessment, monitoring, and clinical documentation.
  • Experience working with pediatric patients and/or individuals with developmental or physical disabilities is a plus.
  • Knowledge of clinical procedures, patient safety standards, and best practices within outpatient, inpatient, or long-term care environments preferred.
  • Excellent communication and interpersonal skills with the ability to build rapport with patients, families, and healthcare team members.
  • Strong organizational skills and the ability to prioritize responsibilities in a fast-paced healthcare setting.
  • Demonstrated ability to work effectively within a multidisciplinary team while providing compassionate, patient-focused care.

  • Must be in good standing on the Nurse Aide Registry in the state of Texas and Misconduct Registry
